Dr. Fox accepts the following insurance:

• Anthem Blue Cross

• American Specialty Health (which includes many BC/BS and Cigna plans)

• Optum Family of Insurances

• VA/TriWest (must have an active authorization from the VA for treatment)

• Medpay, Personal Injury

• Worker’s Compensation

Please confirm that we accept your specific insurance company and plan when making your appointment. Our office will verify chiropractic coverage for said insurance and identify your co-pay/co-insurance.

*Initial visit is not covered by Medicare. Our office is non-participating with Medicare which means we charge the Medicare rate and bill Medicare on their behalf. Patients are reimbursed through Medicare and Supplement Insurance, if applicable.
